To file a program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, (AD-3027) found online at: http://www.ascr.usda.gov/complaint_filing_cust.html, and at any USDA office, or write a letter addressed to USDA and provide in the letter all of the information requested in the form. Summer Program:
Keystone School District is required to inform families of the District how to find the location of Summer Food Service Program sites.  Schools or families can use any of the following methods to locate sites that serve free meals to children during the summer:

•         Call 211
•         Call 1.866.3Hungry or 1.877.8Hambre
•         Text “MEALPA” to 877877 during the summer months
•         Visit the USDA Food and Nutrition Service website
•         Use the site locator for smartphones
